自分も試してみましたがエラー表示もなく、
画像表示されているのでもしかしたらゲーム本体が更新されていない、
Modファイルが壊れている可能性があります。
スチームの「ゲームファイルの整合性の確認」を試してみてください。

自分の方でも実績解除可能なのを確認したので表記を変更しました。
新規公開時に確認したと思ったのですが、
当時は無理だったのが幾つかのアップデートを経て取れるようになったのか、
日本語環境が公式対応されたものによるものか分かりませんが、
まあ、実績ユーザーの方も使用できるようになってるのは嬉しいですね。
該当箇所は本体Ｍｏｄと全く同じファイルですので使用できます。

自分の環境下では普通に表示されているので、
何か他のModを使用していませんか？
ちなみに本体Modも含めアイコン表示関係は変更してなかったと思います。

エラーログに出ていた部分を全て修正しています。
本体Modのみで新規に起動して、出ているエラーログを確認して、
更に新規に当Mod併用でエラーログが出なくなっているのを見てみて下さい。

当Modでの修正関連ファイルには変更が無いようですので、
更新対応は行いません。
Mod『! Immersive Beautiful Stellaris !』のファイルを見てみましたが、
Mod『! Immersive Beautiful Universe !』と全く同じファイルのようなので、
そのまま当Modの修正内容は反映されると思います。
