You can use the fgd file (in the pastebin link) to place them anywhere when making your map.

oooh wait, looking further into the code, there's an entity called "page_spawn" which is used to select the spawnpoints also.

Are the page locations hard-coded? Seems like it, but idk... I would like to make a map for this, but I don't know how it works...
