I've added a new section to the guide called "Enchant roll rules" which details the exact rules - that should answer all your questions :)
Just to make sure i understood you correctly: "Quality" means the distinction between common, magic, unique, legendary and true legendary items? (Its a bit of a confusing term for me, because in PoE "itemquality" is a seperate stat that boosts the affixes on an item, wheras common/magic/rare/unique would be referred to as "raritytiers").
In a nutshell: on a legendary you can have either 1 epic enchant, 1 major enchant and two minor enchants or 2 major enchants and two minor enchants. But you can't have 2 major enchants and an epic enchant, because the epic can't take the place of a minor enchant. Correct?
