(The latin sentence 'Fatum fatis ego perea' roughly means 'I perished by a strange fate').

To get Imperfectus Lacrimatorio you need to kill all the monsters on these rooms and stand on a specific grave for some seconds, until the Penitent One kneels and receives it from a ghost as seen here:

When collecting all four, the item should turn into:

After collecting all parts of Imperfectus Lacrimatorio mentioned before, and having Plenus Lacrimatorio with you, you'll have to head to this room full of grave stones, located in Icebound Mausoleum:

You will then receive Beatus Lacrimatorio:

You'll notice that you now have a golden Bile Flask that fully charges hp and fervor, as shown here:
