Each boss has it's own related achievement, so you HAVE TO do a new game plus to get all achievements on the same file.
This might seem obvious but I have seen nobody state this fact in trying to figure it out myself, so here you go.
To start a ng+ you need to go to the load game screen and follow the directions on ng+ at the bottom of the screen.
