My Mission is Righteous
Complete all stages on Master of Masters difficulty
If you've read the introduction part of the guide, I've suggested start playing the game on Jack difficulty so that you can have most of the skills by the time you unlock Master of Masters. In order to unlock it, you must beat the game on Master Samurai. If you have most of the skills, you won't have any problems with this since it's easier than playing on Samurai without any skills. Make sure to unlock Perfect Dodge, Extended Counter Window, and Attack Cancel before start playing on this difficulty if you want to have an easier time with it. Also always keep some throwables and HP regen items in your inventory.

Deja Vu
Have a conversation with a character in the world
Self-explanatory. Just talk to Rothchild in Aku's Mines and it'll pop up. You cannot miss this.

My Heart is Pure
Use a Kiai Attack
Self-explanatory. Game will tell you how to use Kiai Attack in the beginning. When your Kiai Fire meter is full, unleash it and it'll pop up.

20/20
Defeat 20 enemies using the bow-and-arrow
Self-explanatory. Halfway through the game, you'll have lots of arrows. Just kill 20 enemies with bow and arrow and it'll pop up at the end of the level.

I Have Nothing to Fear
Defeat 1,000 enemies
Self-explanatory. Just keep playing, this will eventually pop up.

Big Bank Takes Little Bank
Collect 100,000 gold
Just keep playing, it will probably pop up on your second playthrough. You're free to spend them so don't worry about that.

As Good As New
Repair a weapon
Self-explanatory. Use Da Samurai's shop to repair one of your weapons.

Close Combat Master
Complete a stage without using a ranged weapon
Self-explanatory. Just don't use any throwable or ranged weapons. Aku's Mines is a perfect level for this achievement.

True Samurai
Complete a stage using only the Sword
You'll probably get this automatically. Just stick to your sword, don't use anything else, including throwables and ranged weapons. Breaking stuff with other weapons is fine, though. This also applies to other weapon-related achievements.

That's the Spear-it
Complete a stage using only the Spear
Wait until you get a spear from the chests or characters then replay Aku's Mines using only the spear-type weapon. Don't forget to set the difficulty to Jack so that you won't have to worry about durability due to low enemy count.

Everything is a Nail
Complete a stage using only the Hammer
Wait until you get a hammer from the chests or characters then replay Aku's Mines using only the hammer-type weapon. Don't forget to set the difficulty to Jack so that you won't have to worry about durability due to low enemy count.

Ace of Clubs
Complete a stage using only the Club
Wait until you get a club from the chests or characters then replay Aku's Mines using only the club-type weapon. Don't forget to set the difficulty to Jack so that you won't have to worry about durability due to low enemy count.

Seek Nothing Outside Yourself
Complete a stage using only Samurai Fist
Just complete Aku's Mines using your fists on Jack difficulty.

Tattered and Torn
Finish a stage with under 10% health
You'll probably get this automatically while playing on Master of Masters difficulty. If you didn't, just go against The Scotsman again, get hit until your HP is very low, then start spamming throwables at him.

Clinch Victory
Defeat a Boss using a Throw
In order to unlock this, you have to learn the Throw skill. I got this in Cave of the Ancient level. Hit the Lava Monster until his HP is really low then use Throw to finish him off. Use your fists to be absolutely sure about that you ended him with a Throw.

Great Interior Design
Defeat an enemy using the environment
There's only one place to get this and it's the Prison Ship. At the point where you've been introduced to small Lazarus creatures, you'll notice the breakable colums that are holding the huge steel creates. Just break the ones in the front, it will cause them to slide and kill one (or some) of the small Lazarus creatures.
Upgrades & Collectibles
Honed Edge
Upgrade any weapon to its maximum level
You can upgrade your weapons at Da Samurai's shop's training section. Upgrade one of your weapons to its maximum level and it'll pop up.

Prepared for Anything
Upgrade all weapons to their maximum levels
See Honed Edge. It takes a long time but you'll eventually get this.

Armed to the Teeth
Obtain all in-game weapons
This one's a bit tricky to get if you don't know how to steal weapons. To steal weapons, you must learn the Throw skill and while using your fists, use throw on a low HP enemy and Jack will automatically steal their weapons. The Lava Monster and Josephine have weapons that you can steal and the some of the regular enemies (beetle drones, robot alligators, bounty hunters, ancient warriors) have them as well. There are also some weapons that can only be found in chests and some of them are exclusive to Da Samurai's shop. Keep an eye out for them and you'll eventually get this one.

Chest Nut
Collect all Items
This one's not difficult, but time consuming since some of the items at Da Samurai's shop don't appear unless you're playing on Master of Masters difficulty. There are bracelets and potions you can buy, you can also find jewels to upgrade your skills inside chests or given to you by characters. Collect all of them and it will pop up. My number one tip would be checking the shop at each location so that you don't miss any of the items.

Jack of All Trades
Learn all Skills
This one's also time consuming. Took me almost 4 playthroughs to get it. Keep an eye out for every chest, speak with every character, collect all the skill fires scattered around the world, and you'll eventually get it. Some items such as Desert Oasis Jewel don't appear out of specific levels so, you may end up replaying Boon's Castle a few times.

Pack Rat
Collect all Collectible Items
This one's worth the trouble because after collecting every item, a secret ending unlocks. See tamarlence's guide for this.

If you have all other achievements, this will automatically pop up. However, it's still bugged since it didn't pop up for me until I restarted the game.

Dog's Best Friend
Speak with Rothchild at each and every location.

Fortune
Open 100 Treasure Chests
Note that you have to unlock 100 DIFFERENT CHESTS! Unlocking the same ones on your second, third or fourth playthrough WILL NOT count towards. It's actually difficult to find different chests each time you play, so I'll provide some well-hidden ones discovered by tamarlence.

Just like Megasonic said above, use an explosive arrow to break open stone doors. Had to have someone tell me this as I had no idea it was a thing. Think there is at least 4 doors you can break through.

The cemetary has these red eye skull statues as well. Shoot through the fence to destroy them and it will lower the fence down for you to collect the goods.

Aku's city is where it gets interesting. After having the Imakandi cut scene where they say you have no choice to be their hunt, look up into the hole above you and there will be some flying robots. Shooting them will spawn a chest. The next spot is really tricky. Im surprised I even found this by myself. After running down the tunnel full of water and pipes you will jump down into the area below and have a fight. After the battle it will open up to where the Scotsman is at. Talk to him then immediately turn around and look at the hole you dropped down from. It will have spawned flying robots. Destroy them for another chest.

In the prison ship in the room where you fight Demongo, if you shoot the three damaged green orbs on the wall it spawns a chest as well.

As for the counter farming, I would say Aku's mines since there won't be many ranged units and they are easy to deal with.

Glide and Slide
Succesfully execute 150 Rushing Guard moves.
Obviously, you have to learn Rushing Guard skill first. Other than that, it's self-explanatory.

Best Offense is a Good Defense
Succesfully execute 150 Counter attacks.
Learn the Counter Attack and Extended Counter Window first then find a suitable enemy for you to easily deal with. I prefer big Ancient Warriors with mallets since their animations are easy to predict.

Quick Reflexes
Successfully execute 150 Parry attacks.
See Best Offense is a Good Defense.

Black Belt
Use 150 Throw moves.
Self-explanatory. You gotta learn Throw first, though.

Escape Artist
Successfully perform 60 Escape Throw moves.
HUGE thanks to JPC for discovering and letting me know about this. I don't have any information regarding the challenge, so I'll just quote him. You have to learn the Escape Throw skill first in order to complete it, though.

Artful Dodger
Successfully execute 60 Perfect Dodge attacks.

Power to Destroy Evil
Use 30 Sword Kiai Attack moves.
There's a bracelet that increases the drop rate of Kiai Fire. Buy it from Da Samurai's shop then start grinding.

Reverse the Spear
Use 30 Spear Kiai Attack moves.
See Power to Destroy Evil.

Hammer Blow
Use 30 Hammer Kiai Attack moves.
See Power to Destroy Evil.

Clubbing like DJ Stylbator
Use 30 Club Kiai Attack moves.
See Power to Destroy Evil.

Shaolin Master
Use 30 Samurai Fist Kiai Attack moves.
See Power to Destroy Evil.

Attack Good
Successfully execute 90 Jump Good attacks.
You just gotta Jump Good and hit an enemy while doing it 90 times. Skill must be learned first. HUGE thanks to tamarlence for this. I wouldn't have discovered it myself if it wasn't for her.

Pest Control
Defeat 180 Beetle Drones.
Self-explanatory.

Demon Slayer
Defeat 180 Celtic Demons.
Self-explanatory.

Crocodile Hunter
Defeat 180 Robot Alligators.
Self-explanatory.

206 Bones, and counting
Defeat 180 Skeleton Warriors.
Self-explanatory.

Grave Danger
Defeat 180 Zombie enemies.
Self-explanatory.

A Bounty on Hunters
Defeat 180 Bounty Hunters.
Self-explanatory.

Buggin' Out
Defeat 180 small Lazarus creatures.
Self-explanatory.

Shadow Meets Light
Defeat 180 Shinobi enemies.
Self-explanatory.

Soul Survivor
Defeat 30 of Demongo's troops.
Self-explanatory. If you've played through the game 4 times on all difficulties, you'll have 28. Simply play Aku's Mines on Master of Masters 2 times since it's the shortest and has a Demongo fight very early in the stage.

Golem to gravel
Defeat 6 Golem enemies.
Self-explanatory. You'll automatically get this on your second playthrough.

Lazarus Butcher
Defeat 6 Lazarus 92 enemies.
Self-explanatory. You'll automatically get this on your second playthrough.
Conclusion
Congratulations! You've finally got back to the past! Still not satisfied? Feel free to try out Free Missions. I was planning to make a guide on them as well until I got on stuck on Free Mission 20 and realized that they've not been playtested properly so, I skipped them and scrapped the idea. Oh? I thought you had to do all of those for the 'all in-game accomplishments' achievement. I guess I misunderstood and wasted a lot of time doing that c r a p. Hahahaha. Thanks for clarifying!

I hadn't really bothered with any of them since they were not related to any achievement and didn't give any significant reward for completion, but Escape Artist in particular was a huge headache and I believe I didn't have more than a single counted attempt.

I realize it has been a year since you worked on this, but I can't find any other information on the internet to help. Did you complete the Escape Artist challenge? I have 2/60 just like JPC's screenshot and I have tried to farm them with most enemies in the game, but can't make any progress. Any insights? If not, I'll probably just throw in the towel on that one.

You can kill him by jumping and attacking, that's how I did it.
