Hey! It's been quite a long time I've been there, but as far as I remember, you can't play the game on one difficulty and then change it to Ultra Violence just for the last level. So you need to play the full game on the hardest difficulty. I've just had a quick look in the Internets and found no info about this being possible as well, but you might want to check more thouroughly if that is critical for you.
Just as a side note - Ultra Violence is not that hard, I don't remember having any Dark Souls moments with being stuck somewhere for hours, so you should be good
Do I need tho play the whole game in the max difficulty level in order to get the achievement?
