解説
Updated May 2021, to work with current version of ModTools
This template is a generic sign that can be easily modified to create unique roadside 'Welcome to' signs for Districts and Cities. I like to use customised city signage so I can easily spot district boundaries ingame when driving around in First Person mode.

The original model on which the sign is based is no longer in the workshop, but as far as I know it was created by someone called Conno (there are quite a few users with that ID!).

And these step-by-step instructions for creating your own signs were made possible thanks to a very helpful YouTube Tutorial created by czardus, on which this Guide is based.

The process may look horrendously complicated when it's all written out like this, but that's only because this Guide is step-by-step, so that even if someone has absolutely no experience of modding, they can easily create their own custom Welcome sign ... once you've created your first sign, you'll be able to create additional signs in no time!

Download from Workshop
Press [Subscribe] above, to acquire this generic sign (it'll be stored under ASSETS in the Content Manager)
ModTools (stored under MODS in the Content Manager)
Find It! (stored under MODS in the Content Manager) - required to access user-created assets ingame
Activate all downloads via the Content Manager

IMPORTANT! Exit and restart the game to properly activate ModTools

Create your sign
[1.1] Using your preferred image editing software, create the image you want to appear on your sign. It must be exactly 454 pixels wide, and 253 pixels high

Hints:
[1] Grab some free clipart or textures off the Internet - for example, search for 'clipart wooden planks'.
[2] To prevent your sign from looking too bright / vivid in full daylight, it's best to choose an image that's fairly dark to begin with ... an example is the Badsmell sign above ... the original clipart is fairly dark, but by the time it's exposed to the full blast of City Skylines' midday sun, it's looking pretty bleached. There again - when the light's right - it looks fine (as in last image). But yeah, generally speaking, the darker the better.

[1.2] When completed, flip the image you've created vertically
[1.3] Then flip the image horizontally
[1.4] Save your image as a png file

Disassemble the Welcome Sign model
[2.1] Select Asset Editor from EDITORS in City Skylines main menu, and choose [LOAD]
[2.2] Select any theme
[2.3] Scroll down to 'Welcome Sign (template)', press [LOAD]
[2.4] Press [Ctrl-E] to open ModTools > Scene Explorer
... scroll down to ModTools and click on the + in front of it
... click on the + in front of CustomPrefabs
... click on the > in front of ModTools.CustomPrefabs
... just past halfway down the right-hand Panel, look for tiny green text PropInfo[] - click on its + (to the left)
... you should now see an expanded line saying Welcome Sign (template)_Data
... click on [Preview]
... at the top of the Preview window, click [Dump mesh+textures] (nothing happens - there's no message or anything)
... close Preview window (the small red box in the top corner)
... close remaining ModTools windows
[2.6] Click cogwheel icon to display Editor Menu (it's in the very top-right-hand corner of your screen)
[2.7] Select [NEW ASSET]
[2.8] Select 'Prop', press [Continue]
[2.9] Scroll down to find and select 'Welcome Sign (template)', press [Continue]
[2.10] On the next screen, click on the tiny folder icon to the left of 'Select a model to import:'

Create your sign using your Image Editor
[3.1] You will now be looking at the Windows File Manager, in the folder
C:\Users\yourname\AppData\Local\Colossal Order\Cities_Skylines\Addons\Import
[3.2] ... there will be two files in that folder:
1569400043.Welcome Sign (template).obj
1569400043.Welcome Sign (template)_d.png
[3.3] Load the png file into your image editor
[3.4] Now copy or move the image you created in Step [1] so that it precisely overlays the white area in the png file.
[3.5] Save the modified welcome sign with EXACTLY THE SAME NAME as the original (in other words, overwrite it). Make sure the new png file is in the same folder as the original png file - if not, copy or move it there

Complete your sign in City Skylines
[4.1] Return to City Skylines Asset Editor.
[4.2] Click on the obj file in the left panel of the 'Asset Importer', and there you will see your new sign.
[4.3] Click [Continue]
[4.4] Click icon to display Editor Menu (in the very top-right-hand corner of your screen)
[4.5] Select SAVE ASSET ... name your sign, fill out the other fields, and Save
[4.6] Quit the Assets Editor

Lastly ...
Deactivate 'Welcome Sign (template)' under Content Manager ASSETS, as it isn't needed ingame
And to save on memory, you could also deactivate ModTools as well

All Done! ... your sign will now be available to you under Content Manager ASSETS. You don't need to upload your sign to the Workshop first (there probably isn't much point if it's custom-built for your City!) ... you can access it ingame via the Find It! menu option.

Looks to need a further update, Welcome sign not showing up on the list in customs prefab similar to what Dannage was saying.

Yo, I found your comment on a video on YouTube and it led me here. This helped me out a ton! I appreciate it. One small note though, from my experience trying it just now, you don't have to flip the picture horizontally. It just needs to be flipped vertically. Thanks for the help!

I'm not using Modtools at the moment, just wanted to add some text using PO only - but that seems to be doing a mirror image of whatever i write, unlike on other road signs (but they already have space taken with town names). Not sre how to go about it.

a little tedious, but works fine for me. Have created 2 (so far) 'gated communities' in my city and created 2 signs. Thanks!

Didn't work for me, failed when I got to the custom prefab bit, as welcome sign didn't appear in the list, even though it was loaded. This is the fourth sign template I've tried to use and failed, so it's obviously me, I must be some kind of stupid.

Jordan, did you enable ModTools, exit game and then launch game again? It won't work completely unless the program launches with it already activated (found through trial and error).
