Preface
This guide contains directions and screenshots to each of the game's hats, needed for the Mad Hatter achievement.

Since the hats can be found in locations including the game's finale, there are moderate spoilers contained - read at your own discretion.

Note that you'll need to have completed the game at least once in order for hats to appear.
Act I - Discovery
#1 - A Strange World (Baseball Cap)

In the early parts of this chapter, you are required to pull a lever that causes a nearby wall to slide inwards in order to progress. In order to reach this hat, you'll need to pull the level just enough so that the top part of the wall doesn't withdraw all the way - this will allow you to cross this "bridge" when you climb the nearby stairs, to where the Baseball Cap is located.

#2 - A Strange World (Soccer Ball)

At some point in this chapter, one of the buildings you come across will sink into the ground as you jump on it - allowing you access to a nearby cave. The Soccer Ball hat is located in the lowest wooden platform in this cave.

#3 - Bridging The Gap (A Fish)

As soon as you reach the bridge puzzle (where you need to move cardboard houses around in order to cross the nearby gorge), jump into the water to unlock the Fish hat.

#4 - Bridging The Gap (Fish Bowl)

In the same location as hat #3, but this time you need to place the houses diagonally - towards the lone white block floating to the left of the entrance. Reaching that block will unlock the Fish Bowl hat.

#5 - Rise to the Occasion (Old Boot)

As soon as you climb the stairs and find Alejandra (the little girl) sitting in front of you, head to the left of and around this building (make sure you don't step into the chalked outline and trigger a level transition) - the Old Boot hat is hidden behind the building.

#6 - Rise to the Occasion (Tiara)

After acquiring Lula and completing the first few jumps, you'll be directed to climb a staircase to access the next part of the game; instead, head to the left of the stairs and across the catwalk to find the Tiara.

#7 - Along for the Ride (Straw Hat)

After you solve the sewage pipe puzzle, one of the pipe's ends will bend downwards as you approach, allowing you access to the room below. The Straw Hat is located beneath the ramp on which you land after exiting the pipe.

#8 - Forbearing Shadows (Wizard Hat)

In the plaza where you need to solve the white blocks puzzle, there's a huge pipe covering the far end of the area - simply walk beneath it and follow the scaffolding to the Wizard's Hat.

Act II - Protection
#9 - Cozy Digs (Plastic Jug)

As you descend the spiralling ramp, there should be a roof to your left that you can jump on - from here, follow the rooftops to a larger gap and the Plastic Jug hat.

#10 - Deception (Striped Cowboy Hat)

In the area where you need to trap Monster inside a cage, there'll be a ladder drawn on the wall that you can climb; at the top, turn around and jump across the rooftops to where the red pipeline is. Jump atop the pipe and follow it to the Striped Cowboy Hat.

#11 - Mmm, Tasty (Skull Bandana)

After you activate the first set of chalk cogs on the wall, there will be a building you can split into two in order to create a platform for Quico to use - the Skull Bandana should be in the alley to the right of this house.

#12 - Bend Them To Your Will (Golden Crown)

During this level's puzzle sequence (where you need to build a bridge by activating keys around the area) you'll come across a very tall stepladder; the nearby rooftops contain a doll wearing the Golden Crown.

#13 - Bend Them To Your Will (Melon Helmet)

In the same puzzle, once you've found all of the bridge blocks, extend the bridge outwards and to the right - there should be a single white block floating in the distance. Using the bridge, climb atop it and across the gap to find the Melon Helmet.

#14 - Protection (Pasta Strainer)

In the puzzle where you need to lift part of the ground to make a ramp and reach the floating key, simply enter the small room beneath the lifted ground to find the Pasta Strainer hat.

#15 - White Mountain (Wooly Horned Helmet)

When jumping down to the very first rotating segment in this area, make sure you land in the slightly raised flowerbed on the right-hand side of the segment. There is a doll here, wearing the Wooly Helmet.

#16 - White Mountain (Tire)

In the same location as hat #15, you'll be climbing a series of floating blocks to reach the key needed to progress - a second floating platform behind the key holds the Tire hat.

Act III - Friendship
#17 - Multiverse (Top Hat)

Once you reach the area with the chalked soccer field, pull the middle lever (which releases frogs into the field), then head behind the red wall to find the Top Hat.

#18 - Relax, Why Don't You (Chef's Hat)

At the end the sequence where you move Monster around on the centipede, use Monster's belly to hop over the wall to his left. The courtyard behind contains the Chef's Hat.

#19 - A Builder Of Bridges (LEGO Block)

At the end of this sequence, as soon as you cross the wooden bridge you should see this hat to the right of the exit.

As Steam user q3muyq3 notes, you can alternatively load up Chapter IV: "Anger Management - Over The Inferno" and backtrack to the previous section using Lula to get this hat.
Act IV - Anger
#20 - Anger Management (Paper Bag)

Near the puzzle where you use a block lever to push Monster inside an anger-draining trap, you can follow the right-hand wall to the Paper Bag hat.

#21 - Anger Management (Cooking Pot)

As soon as you are reunited with Lula again, use the various floating objects to reach the area's exit; instead of leaving this area, turn to your left and you'll see a lone house that's just within reach of a double-jump. Atop this building lies the Cooking Pot hat.

#22 - Over The Inferno (Flower Pot)

At the end of the platforming sequence, the final platform before reaching the capsized centipede contains the Flower Pot hat.

Act V - Liberation
#23 - Euclid Is Wrong (Rasta Hat)

At the right-hand side of Monster's first statue, there is a flowerbed; within it lies the Rasta Hat.

#24 - Growing Up (Birthday Hat)

After completing the final puzzle (where you create replicas of Alejandra to lure the Monster to the final part of the level), while following the wooden bridge you should see the Birthday Hat on a floating tin sheet to your left.

#25 - Growing Up (Monster Hat)

Just before sending Monster over the edge, you can find the Monster Hat behind the nearby tree.

Thanks everyone for all the comments!

Re: Missing pictures, this seems to be something on Steam's end, as all the pictures are hosted by Steam via its screenshot feature. Hopefully it'll resolve itself because I've tried editing the guide to re-insert them and they still show up as broken (you can still see them if you click on the broken picture icon, though)

Re: Hats not appearing - as mentioned in the preface, the hat "feature" in this game unlocks after you've beaten it at least once, so if you've already done this, maybe the game didn't save properly at the end. I'm sadly not aware of any issues that'd prevent hats from showing up, but I'd check in the Steam Discussions section for any workarounds.
